Learn How You Can Help Your Community Members
Community Health Workers (CHWs) are trusted advocates who work across diverse settings to break down barriers to care, address systemic inequities, and support the health and well-being of our most vulnerable communities.
What Do Community Health Workers Do?
- Promote wellness by helping people adopt healthy behaviors and make informed health decisions.
- Serve as a vital link between the community and health/social services, ensuring that everyone—especially those in underserved or marginalized populations—can access the care and support they need.
- Build trust, dispel misinformation, and connect people to critical health services, particularly in communities of color, rural areas, and other settings where barriers to care are greatest.
- Increase health knowledge and self-sufficiency through outreach, community education, informal counseling, social support, and advocacy.
- Improve health outcomes by supporting chronic disease management, reducing hospital readmissions, and promoting mental health and preventive care.

NEON is a connector, innovator, and expert resource
Our mission is to support individuals, communities, and systems to collectively promote equity, health, and well-being at home and beyond. We are connectors, innovators, and experts. We connect people and organizations to one another and to resources they need to meet their goals. Since 2012 we have been training traditional health workers and other front-line staff in behavioral health, social services, and other settings in the knowledge and skills necessary to provide quality service, maintain professional boundaries, and practice self-care.
Our History
NEON is a 501(c)3 nonprofit health collaborative originally serving Union, Wallowa, and Baker counties in Northeast Oregon and recently expanded into Umatilla and Malheur counties. NEON launched in 2004 from a small group of individuals who recognized the value of community collaboration in ensuring the health of our residents.
